New patient forms and cancellation policy
It is our office policy to have a credit or debit card on file to reserve your appointment. We have an office cancellation policy - there is a 40.00 charge for cancellations without 24 hours notice. We have this policy in place as our dietitians reserve your appointment time just for you and work by appointment.
Once you schedule your appointment and the appointment is approved, a link will be emailed to you to complete your forms in our HIPAA compliant patient portal. Please complete the forms emailed to you within 24 hours notice of scheduling. Your appointment may be cancelled if the documents are not completed prior to the appointment. If you do not receive your new patient forms, please contact us so we can ensure you receive them.
Patients with Aetna Insurance
Aetna limits the time we can spend with you to 1 hour maximum per session. Due to Aetna limiting our time, after the initial session your dietitian may request to see you the following week if your schedule allows to continue your initial consultation and develop your individualized plan.
What can I expect from my appointments?
This is the dietitian's opportunity to get to know you and your lifestyle and it is your opportunity to let your dietitian know exactly what you’d like to get out of working with them. As integrative functional dietitians, we focus not only on diet and nutrition but also your lifestyle, stress and hormones, movement and fitness, sleep, and mental, emotional and spiritual well-being. We will perform a comprehensive health history and learn about your lifestyle habits and diet. With this information, we will develop an individualized lifestyle plan for you based on your goals and needs. Our goal is to help you find and understand the causes of your health challenges; we may offer functional testing in addition to your nutrition counseling. If essential, we may recommend in-depth laboratory testing of blood, saliva, urine, breath, and/or stool. This testing is more comprehensive than traditional laboratory testing and comes from specialized laboratories. Functional tests are not covered by insurance; they may be recommended but they are not required to work with us!
Communication
To maintain the utmost quality of care to our patients and to respect that our dietitians work by appointment, please reserve extensive questions to your appointment time. The dietitian may share their email with you for quick questions or lab results. For other matters, they will reply to you during their scheduled working hours or during your appointment time. For immediate questions or concerns, you may contact our office so we can answer your questions and concerns in a more timely manner.
